Phase One — Pre-Entry Foundation · Module 7 of 24 · 90 minutes

Housing, transportation, employment, support networks, and first 90-day planning.

What you will be able to do

  • Consolidate every prior module into one written release plan.
  • Confirm housing, transportation, and a first move toward employment.
  • Identify a support network for the first 90 days.

Why this matters

Everything in Phase One comes together here. A release plan turns six modules of preparation into a single, usable document you can hand to a case manager, a parole officer, or yourself on day one.

Reality check: People rarely fail reentry because they lacked information. They fail because the information never got organized into a plan they could actually follow under pressure. This module is where that changes.

Activity

Release Plan Build — using Appendix E (90-Day Success Plan), consolidate housing, transportation, employment first-step, financial plan, health plan, and support contacts into one document.

Scenario

Release day arrives and the plan that existed only in your head falls apart under real conditions — no ride, no clear address, no one expecting you. A written plan is the difference between a setback and a derailment.

What would you do first?

Reflection

  1. What part of your plan feels most solid? What part feels shakiest?
  2. What’s the very first thing you’ll do in your first hour out?

My action plan (pick at least one)

  • Complete a written 90-day release plan covering housing, transportation, work, money, health, and support.
  • Confirm your first 72 hours specifically — where, how, with whom.
  • Share a copy of your plan with your case manager and one support contact.

Resources

  • Appendix E — 90-Day Success Plan
  • Appendix C — Housing Readiness Assessment
  • Renewed Ambition case management team

Completion checklist

  • ☐ I have a written 90-day release plan.
  • ☐ My first 72 hours are specifically mapped out.
  • ☐ A case manager and a support contact both have a copy of my plan.

Milestone: A complete, written release plan exists and has been reviewed with a case manager.
