Phase One — Pre-Entry Foundation · Module 1 of 24 · 90 minutes
Obtaining identification and vital documents.
What you will be able to do
- Identify which of your vital documents (birth certificate, Social Security card, state ID) are missing.
- Understand the request process and timeline for each document.
- Leave with a written plan to secure missing documents before release.
Why this matters
Your ID is your access pass to the free world. No ID means no job application, no bank account, no lease. The request process for a birth certificate, Social Security card, or state ID often has to start while you’re still inside — waiting until release day costs you weeks.
Reality check: Most people don’t lose their footing after release because of one big mistake. They lose it in week one, standing at a DMV counter with no birth certificate, no way forward, and no plan B.
Activity
Document Inventory Worksheet — each participant lists every vital document, marks it Have / Don’t Have / Not Sure, and identifies the one next step for each missing item (who to call, what form to request, what it costs).
Scenario
It’s your first day out. You hit the DMV for an ID, but they ask for a birth certificate you don’t have. No ID means no job application, no bank account, no apartment lease — nothing moves.
What would you do first?
Reflection
- Which missing document worries you most, and why?
- What’s one call or form you can complete this week?
My action plan (pick at least one)
- Request pre-release ID forms from your facility counselor.
- Write down every address you’ve lived at (required on most forms).
- Confirm fees and payment/waiver options for each document.
Resources
- U.S. Social Security Administration (SSA.gov) — Getting a Replacement Card
- National Vital Records / CDC (CDC.gov) — Ordering a Birth Certificate
- State DMV — REAL ID requirements and fee waiver options for justice-involved individuals
Completion checklist
- ☐ I know which vital documents I have and which I’m missing.
- ☐ I’ve submitted a request for at least one missing document.
- ☐ I know where my documents will be kept safe after release.
Milestone: At least one missing-document request is mailed or submitted before release.
