Phase Two — Community Reentry · Module 10 of 24 · 90 minutes

Making the most of your case manager relationship instead of avoiding it.

What you will be able to do

  • Understand the role of a case manager and what to expect from regular meetings.
  • Build the habit of bringing real problems to case management before they become crises.
  • Set and track goals collaboratively with your case manager.

Why this matters

A case manager isn’t supervision for its own sake — it’s the one standing appointment in your week built entirely around your progress. Residents who use it well move faster through every other phase of this program.

Reality check: A lot of people come from systems where check-ins meant surveillance, not support. It’s normal to be guarded at first. The residents who do best are the ones who test that guardedness against what actually happens in the room.

Activity

Goal-Setting Session — with your case manager, set two 30-day goals and log them on Appendix I.

Scenario

A resident hits a setback — a missed job interview, a family conflict — and doesn’t mention it in case management for three weeks. By the time it surfaces, it’s a bigger problem than it needed to be.

What would you do first?

Reflection

  1. What’s one thing you’ve been avoiding telling your case manager?
  2. What would ‘using this relationship well’ look like for you?

My action plan (pick at least one)

  • Attend your scheduled case management meetings consistently.
  • Bring one real problem or question to each meeting, even a small one.
  • Track your goals on Appendix I between meetings.

Resources

  • Appendix I — Case Manager Progress Tracker
  • Renewed Ambition case management team

Completion checklist

  • ☐ I know my case manager’s name and meeting schedule.
  • ☐ I have at least one active goal logged with my case manager.
  • ☐ I’ve brought a real concern to a case management meeting, not just small talk.

Milestone: Two consecutive case management meetings with goals set and reviewed.
