Phase Two — Community Reentry · Module 16 of 24 · 90 minutes

Building a real relapse prevention plan and owning your recovery.

What you will be able to do

  • Identify your personal relapse triggers and warning signs.
  • Build a written relapse prevention plan.
  • Understand accountability as a tool for staying on track, not a punishment.

Why this matters

Recovery isn’t a single decision made once — it’s a system of daily choices, supported by a plan built before you need it. A relapse prevention plan built in a calm moment is far more useful than a decision made in crisis.

Reality check: A setback doesn’t erase the progress that came before it. What matters most is what happens in the hour after a setback — whether it’s met with a plan and support, or hidden and left to grow.

Activity

Relapse Prevention Plan Build — identify triggers, early warning signs, coping strategies, and a support contact list; commit it to writing.

Scenario

A resident feels a familiar pull during a stressful week. Because they built a relapse prevention plan in advance, they know exactly who to call and what to do instead of facing the moment alone.

What would you do first?

Reflection

  1. What’s your plan for the first hour after a high-risk moment?
  2. What would make it easier for you to ask for help before a setback, not after?

My action plan (pick at least one)

  • Write a personal relapse prevention plan covering triggers, warning signs, and coping strategies.
  • Identify at least two people you can call in a high-risk moment.
  • Know exactly what to do and who to tell if a setback happens.

Resources

  • SAMHSA National Helpline — 24/7 substance use and mental health support
  • Renewed Ambition Life 2.0 Modules (Substance Abuse & Awareness)
  • Local peer recovery support meetings

Completion checklist

  • ☐ I know my personal triggers and early warning signs.
  • ☐ I have a written relapse prevention plan.
  • ☐ I know who to call and what to do if I’m at risk or experience a setback.

Milestone: A written relapse prevention plan is complete and shared with your case manager.
